A lot of my providers see patients for follow up visits of concussions and other injuries. Of course, the first encounters have the most information, including the external cause of injury. Most of the subsequent encounters do not have documentation of the external cause of injury.

I have been querying the physicians to document the external cause of injury or document that further details can be obtained from "x" date of service. Am I right in doing this? Some of the physicians question why they have to write it again when they documented it in the initial encounter. I'd like to make sure I have a solid and definite response for when they question me - unless what I have been doing is unnecessary.
