I work for a Physiatrist who is board certified in Physical Medicine & Rehabilitation and Pain Management. For 2013, in what sequence should these codes be placed to receive the maximum reimbursement for the following CPT and ICD-9 Codes:
99243 723.4, 354.0, 354.2, 353.0, 729.5, 729.1
95860 723.4, 354.0, 354.2, 353.0, 729.5, 729.1
95909 723.4, 354.0, 354.2, 353.0, 729.5, 729.1

Any help would be appreciated!
