Can any one tell me when coding excisional debridement on the out patient side (CPT Coding) does your facility require that the documentation must stated:



A description of the procedure as "excisionaL"

A description of the instrument used to cut or excise the tissue (e.g., scissors, scalpel, etc.)

A description of the tissue removed (e.g. necrotic, devitalized, etc.)

The appearance and size of the wound (e.g. down to the fresh bleed tissue, 7cmx10xm, etc.

The dept of the debridement(e.g. to skin, fascia, subcutaneous tissue, etc.)



in order to code for excisioinal debridement. I know the CPT book just states code to the deepest layer of tissue removed.

When I review my provider's notes, the only thing I look for is size and depth. It is also possible to use more than one debridement code if there are multiple wounds with varying depths.
 
How do you calulcate though for the sq cm?
An example I have is 1.5 cm wide and .05 cm deep.
