Wiki Excision of skin flap help

Would this be included in the E/M or a separate procedure? My provider states it as a skin flap debridement?

I&D: General
Site: right great toe
Procedure description: other (Skin flap debridement)
Procedure prep: other (Soap and water)
Procedure tolerance: good
Procedure details: Both the wound and the toe were soaked in soap and water. Skin flap to the tip of the right great toe was cut off with sterile scissors. Patient tolerated this well without anesthesia. Bacitracin applied to the tip of his toe with a Band-Aid covering.

This depends on the actual E/M dictation. Was this a planned procedure in which the medical decision making was made at a previous visit for this debridement? Is this a new complication being evaluated for the first time by your provider? Without more details this is hard to say definitively.
