Wiki Excision of Mass - Do I have a billable procedure

Do I have a billable procedure if the physician does not document the excision size?
I've got an op report that shows a neck mass was removed from just below the subq tissue but no size was reported. I've heard both ways: code the smallest size possible or you can't code procedure if no size is documented. What's your thoughts?
