Wiki excision of gouty tophi

doc did an excision of gouty tophi of the right elbow the tophus was excised taking the olecranon bursa with it down to the bone. would you use 24076 or can we use 24000? i am at a loss. thanks for any help.

I know this is a super late reply, but in case anyone else is having this issue and runs across it like I did for a similar case....

The CPT code to be used is 24105 Excision, Olecranon Bursa. You would add a laterality modifier to it (in this case RT).

The excision of gouty tophi (24000) is a column 2 NCCI edit and can be overridden with a modifier, but in this case it is included in CPT 24105.
