If a provider documents: 'On exam there is perhaps some fading in the color of the granuloma annulare on his flanks but overall it is still quite prominent, still annular and violaceous on the dorsal hands' can the doctor be given credit for both upper extremities, abdomen and back or not at all because these elements were not specifically documented as such? Thanks for your help.

I would allow credit for both upper extremities and either the back or abdomen. Since there is not much documentation about where on the flanks the condition is, I would allow one or the other.
