Wiki Exam Question.. Please Help

I have a question and being argued with I was wondering if anyone can help.

If in the exam under Consitutional the provider states: The patient is alert and in no acute distress, appears well nourished.

Would you give credit for this under.. Just Constitutional or Constitutional and Neuro or Constitutional and Psych.

Only Constitutional

I will consider them all under Constitutional.

I would only give credit for constitutional.

If provider had documented alert and oriented x3 (person, place, time) then I would give credit under psych also.
