Does anyone bill for euflexxa? If so can you tell me if the commercial and medicare plans pay?

Euflexxa injections

Medicare pays with a diagnosis of osteoarthritis only-refer to the Medicare LCD on viscosupplementation/hyaluronic acid injections. Medicare plans will follow Medicare guidelines on these injections
Commercial plans do pay-check with each individual plan to see if an authorization is necessary.
