If pt has procedure done and returns a month later with stones, can I charge an office visit with Mod -24 ???

You should be able too. As long as you can prove these stones are not related to the same problem that the ESWL was done for. It may be denied but you should be ready to appeal it with the office notes showing they are seperate.
