Provider does a left craniotomy transmastoid & middle fossa approach for repair of CSF leak. Does 61591 fit?

Hello,

I found the following under the lay terms for 61591.

This code is reported for a surgical approach procedure during which the physician accesses the middle cranial fossa using an infratemporal (below the temporal fossa) pre–auricular (in front of the external ear) approach, sometimes called the transparotid approach, to expose a lesion or defect at the base of the skull. The approach can also include a mastoidectomy, resection of the sigmoid sinus and/or decompression/mobilization of the auditory canal or petrous carotid artery. The selection of a particular approach will depend on the location and size of the lesion. Report other surgeries, repairs or reconstructions separately.

Clinical Responsibility
The physician utilizes infratemporal fossa or area behind the auricle of ear to access the hollow region (bounded by sphenoid and temporal bones) containing tissue defect. Multiple techniques like removal of the mastoid portion of posterior temporal bone and sigmoid sinus, stretching (with preservation) of seventh cranial nerve and petrous segment of internal carotid artery, and release of pressure from ear canal (external auditory meatus, external acoustic meatus) may be performed to access the diseased tissue.
 
61591 only describes the approach. It does not cover the closure which the Otolaryngologic Surgeon performed.
CPT section notes says:
The repair/reconstruction procedure(s) is reported separately if extensive dural grafting, cranioplasty, local or regional myocutaneous pedicle flaps, or extensive skin grafts are required.

For primary closure, see the appropriate codes (ie, 15730, 15733, 15756, 15757, 15758).

Since alloderm was used for the closure and the size was 2x4cm, I would code 15275 in addition to 61591 - Application of skin substitute graft to face, scalp, eyelids, mouth, neck, ears, orbits, genitalia, hands, feet, and/or multiple digits, total wound surface area up to 100 sq cm; first 25 sq cm or less wound surface area
