Wiki Endoscopy by Capsule

I have been just started coding the capsule study. Going by the LCD, are they wanting the first code to be the v45.89 plus other ICD-9 codes. When I look up the V code in the book it says that v code has to be the 2nd dx only. I look at the LCD as they are wanting the v code first. Any help will be sooo useful. Thanks in advance

Our LCD also states that V45.89 must be listed as primary to indicate that the pre-requisites were completed. We have been billing these out with the V code primary and don't have any problems.
