Wiki Endo Families (billing vs payment)

I am a little confused about the billing and payment of the "families". What are the rules for the billing and payment of the codes in the families? Are there any resources or web sites that can explain the procedures for the billing and coding of families? I am not getting paid what I would have expected from some payors.
