I need some help with an operative report. The physician performed a common femoral endarterectomy with patch closure and then incised into the patch to place the femoral popliteal bypass graft.
"Attention was then paid to the to the left femoral area. Again, proximal and distal control on the common femoral artery was achieved. The arteriotomy was made and edarterectomy was performed. There was good tapering distally into the profunda and then this endarterectomy was closed with Hemashield patch in both directions with 6-0 Prolene suture allowing backbleeding and forward bleeding before tying the last stitch. Again with control on that femoral artery, a 1 cm incision was made in the patch and the proximal anastomosis of the femoral popliteal Propaten graft was completed with running 6-0 Prolene suture, again allowing forward and backward bleeding before tying the last stitch."
Would you code for the endarterectomy or consider it bundled with the graft?
