Define Observer (EMR requirements)
Both CMS and Texas Medical Association state: date and legible identity of observer. My interpretation of this is anyone who enters into the medical record. In my case, I am trying to show proof the ancillary staff entering ROS and PFSH should be identified along with the date they entered the information.
This has been somewhat frustrating, any input I can get or additional resources would be helpful. I did call Novitas I was told my interpretation is correct, but I do not have anything in writing.
