Billed Medicare, CPT 95886, 95908 (in this order). Date of service is 1/15/13.

95886 was denied "the related or qualifying claim/service was not identified on this claim", "add-on code cannot be billed by itself". The qualifying service is procedure 95908 which was allowed/paid.

Your add on codes should never be first listed. That could be the problem
