When EMG - CPT 95886 are use for more than one extermity does any modifer required or it is coded per units, such as BL thighs >5 muscles tested will be CPT 95886 x2 OR CPT 95886x1 & CPT 95886x1.

Most of our payers want it on one line:

95886 x 2

But we have a couple smaller payers that want us to change it to:

95886 x 1
95886 x 1 - 59

It was a trial-and-error run for us when these codes changed a couple years ago.
