Billing for EKGs, readings only-Provider bills for 3 EKGs, in one day. @ of the EKGs are done earlier in the day, and the referring physician is Dr. A, later in the day another EKG is performed and referring physician is Dr. B, claims are submitted to payer(s), modifier 76 is added to the 3rd EKG and submitted as two separate claims, due to the different referring physicians. Payers almost always deny the second claim as duplicate, this is not a duplicate claim. Has anyone experienced this issue? Tips and or tricks to have claim paid right the first time? What is the rules in relationship to the referring on the claim?
