What are the documentation requirements for 93005 performed by the physician in the office during a follow up visit? Is an order for the procedure sufficient? The ordering physician is not interpretating the EKG but only performing the procedure. The article in AAPC Healtcare Business Monthly is a little confusing to me on that specific behalf.
