When the physician inserted the scope he accidentally inserted the scope into the trachea which caused hypoxia in the patient - the scope was withdrawn, hypoxia resolved and then inserted properly and the EGD was completed without any further issues. How would you code or would you code the accidental insertion into the trachea?
