Hello, please help with this situation:\
A PA has documented critical care time in her dictation. There is an addendum that states that the attending Dr. has participated in the EM. Not specifically the critical care. It is also signed by attending. Would you bill the CCtime for the Attending or Pa. (OR)Also, do you think it would be best to send the note back asking for clarification from Attending if he participated in the CCtime? (or) Bill 99285 with giving credit to the attending so we can avoid getting reimbursed less for billing cctime with giving credit to the pa. Just wondering ...need an opinion.
 
No/ Return the Chart

I would return the chart to the physician. Since it is a shared service you cannot combine the PA and Doc for CC (unlike other E&M). However if the Doc documents 30+ minutes of CC time, meaning they really provided that amount of CC time in addition to the PA, you can code for the doc's CC time. If the doc didn't meet the requirments for CC Plan B would be combining the PA/Doc for the approriatte ED level.
