What specific documentation is required to bill for the PW/CW Doppler and color flow Doppler ( 93306 or +93321, +93325)?

Sometimes the providers say "93306" in the Procedure performed, but they don't mention color flow in the report, they don't have Doppler measurements, or both.

Sorry if this is dumb, I am new to this!
 
Color flow

I asked our physician and he said color flow is included in the doppler study. The color flow is part of the doppler study that studies the direction of the flow, red means flow is towards the transducer and blue if away from the transducer.

In our report, our heading is 2D echo, Doppler study, conclusions.

Our physician said that typically it is understood in the specialty that color flow is part of doppler.

For complete echo study we bill 93306.

For limited echo study we bill 93308, +93321, +93325 if appropriate.
