Wiki ECG Monitoring - baselines billable?

I work for a remote cardiac monitoring company and no one seems to know if baseline testing, cpt 93012, is billable or not. Coding book says - telephonic transmission of post-symptom electrocardiogram rhythm stip(s), 24-hour attended monitoring, per 30 day period of time; tracing only.

So, a couple of scenarios

1. Baseline completed, but didn't continue service for the 30 days
2. Baseline completed and service continued for 30 days
3. Baseline done, but allergic reaction to adhesive pads.

Any clarification someone can offer will be greatly appreciated.
 
Cardiac Monitoring in the ED

I am a facility coder for a hospital Emergency Dept. Can I use 93041(Rhythm ECG, 1-3 leads: tracing only) to bill for cardiac monitoring done in the ED by the ED nurses?
