Wiki E&M with PT same day

What modifer will be most approicate to use. Billing for E&M and PT same day differ provider Name, same NPI, same tax ID. I have use 25 on the E&M was was paid but could this be an error? Could it be 59 on the PT CPT's
Modifier 25 ?Significant, separately identifiable evaluation and management (E/M) service by the same physician on the day of a procedure

The payer may not allow incident to billing of PT under the physician's provider id. I would suggest checking the payer policies, if you can. If the 25modifier denies chances are the 59 will also deny.
