Wiki E&M With Injection

Does anyone know if there are payors out there that will make payment for an E&M with an injection when the patient is established and has been seen within the last 3 years?

Modifier 25 on E&M visit, injection fee, injection. That's all you really have to do. But make sure to review the billing guidelines for the payor you're looking at.
If you're not billing injection fee(because patient maybe only came in for immunizations and it's included as office component), E&M with injection doesn't need any modifiers and is payable by most payors.
