Mom is seen with her twin girls for routine health check, (twins 2 wks of age) by primary. Resident physician then takes over for breast feeding consult.
Time spent with all three simutaneously, 45 mins between the health check and the consult. Three 99214 office visits were billed. I believe this to be inappropriate. Thoughts?

To bill three independent claims there needs to be three separate notes with three different history, exam, decision making. You cannot have one note suffice for three diffentent encounters.
