Our physician seen a patient who came in with hidranitis suprativia, very painful in a very painful area. Patient has been dealing with this for several months and being the area it is in no one would treat her. Long story short, our doctor did treat her took him 20 minutes to infiltrate the area with tumescent anesthisia, an hour for the procedure itself, total time spent with patient was 2 hours. Can I bill 99215 with 99417 x4? is this possible to do?

You should also bill the procedure becasue you can't combine E&M time and procedural time in order to use the prolonged service coodes. That's been made very clear by both AMA and CMS.
