Is it customary to bill an E/M code in addition to a procedure, such as a wart removal? Or is it ever appropriate to bill only the procedure?

That depends are they in for just the wart removal as a scheduled procedure? Or was there a seperate evaluation that might support the use of modifier 25?

What would be the scenario either way? I don't have that info yet.

If the patient was scheduled for something different than the wart issue and while in the visit said "oh and by the way I have this wart, can you remove it?" you can bill an E&M visit with the icd-9's to support the original reason the patient was seen with a -25 and also report wart removal with associated icd-9 code to support.
