If a physician orders chemotherapy to be started in 2 weeks would you include (Drug therapy requiring intensive monitoring for toxicity) or (prescription drug management) as the chemo is not performed that day. MDM high or MDM moderate?

I would consider it high regardless of when its to be performed. If the provider made that decision today, then his decision making (MDM) today is high.

I too agree. It's about the decision, not the actually timing of the therapy/procedure, etc.
