Phycians did surgery on LT knee. Pt comes back in the global period for follow-up visit LT knee is doing great, but pt bring up that his RT knee has been really bothering him physician exams RT knee orders x-rays and decided to order an MRI on the RT knee. This is all in one office note. New condition? Can we added the modifier 24 to this visit and send to insurance carrier?

Please any guidance on this will be appreciated.

The RT knee pain is completely unrelated to the surgery performed on the LT knee, so yes, you are justified in billing an E/M with mod 24.
