I know that Medicare will not accept "noncontributory" as documenation in the HPI but what about private insurances? My gut is to follow the same guidelines as Medicare but I want to be certain.
Oh and what if you have a pt that can't speak due to severe brain injury recently sustained so physician documents "unotainable" for ROS. Does he get credit for that?
