Does anyone know of a helpful class/webinar/seminar to better understand physician coding and documentation for hospital rounds (99221-99223, 99231-99233, 99251-99255, 99291-99292, 99217-99226, 99234-99236)? For example: My most recent issue has been asking the doctor to document two H&P's for a patient seen in observation the first day and admitted the next.
