A patient comes into the practice with knee pain or shoulder pain and Dr does a history, exam, and medical decision making. Reviews xrays and will discuss options witht he patient. The patient decides to have a joint injection. Would you charge an E/M with a 25 modifier and the procedure on this first visit? Thank you in advance.

CCI edits are very clear. E&M with a minor procedure (0-10 day global) is bundled into the procedure, unless something very separate and significant is done outside what would normally take place prior to the procedure. I'd say in this case, you'd bill only the joint injection, because what the provider did is definitely within the scope of a pre-injection evaluation.
