Patient seen first time Doctor X for diverticulitis and a change in bowel habits. Now Doctor X requested colonoscopy to another Doctor Y (different doctor's office). What's the E/M code and appropriate modifier?

There isn't an E/M code for colonoscopy, or for any condition for that matter.

You need to have documentation to assign an E/M code.
