You say this was for Pessary maintenance. If you are providing maintenance such as cleaning you should bill the E&M only. The 57160 is for the initial fitting and for a refitting if it is necessary due to needing a different size. Routine cleaning and maintenance is just the E&M code. If this was the initial fitting then did you use a 25 modifier on your E&M?
