Wiki E/M Check List Form

I went to an AAPC workshop for E/M coding. There was a checklist form that was given at the presentation. Unfortunately, I cannot find my copy and cannot find one online. Is there a way to get another copy. We are trying to do an audit and I am really could use the help. Thanks.

That worksheet is based on the 95 guidelines, what if the provider uses the 97 guidelines? I have always had to make my own for that, I just wondered if you had any better resource for the 97 guidelines.
