When choosing the E&M based on medical decision making is it required for the provider to also document the time? For behavioral health--is time required for E&M and/or counseling codes? Is time documentation required for telehealth? Our providers are basing their E&M selection on medical decision making only as that is what benefits them most but are just wondering if they also need to document their time. We have family practice and behavioral health providers and they do both in person and telehealth visits.

This question can be answered by reading the first page of the new E/M guidelines. It clearly states which codes MDM applies to.
Thank you for replying. I do know which codes MDM applies to--I am only asking if time is also required to be documented if choosing the level based on MDM. For example--do they have to document both but then select the code based on whichever benefits the provider or can they choose to only document one--MDM or time.
