Can you bill for a pessary 57160 if at the initial visit it doesn't work? My doc tried to insert a pessary for a patient but the patient couldn't keep it inserted and complained of pain, and it was a failed attempt, so can we still bill for the pessary (maybe with a modifer) or just the e/m.
