Wiki E/M and non-mature fistula

If patient comes in with non-maturing fistula and uses a IJ cathether for dialysis and has no complaints of any symptons - is this E/M included in the surgery knowing the interventionalist is going to do a fistulagram/ultrasound? (Since nothing has changed since that patients first visit)

If the fistulagram was scheduled, what kind of E/M was done? Was it a separately identifiable service or was it a minimal H & P and consenting of the patient prior to the fistulagram?
 
Yes it was a minimal e/m. The hpi just said non maturing fistula. The pa then ordered ultrasound and found stenosis. Then followed with 36147 35476 & 75978.
