Wiki E/M and Gentamicin infusions - Please respond

A patient sees Dr. and it is decided they are a candidate for
gentamicin perfusions.
1st Encounter: Dr. charges E/M and perfusion.
2nd Encounter: Dr. charges E/M and perfusion
3rd Encounter: Dr. charges E/M and perfusion

Should there be E/M charges on the encounters after the first visit?
I'm hearing 2 different answers and need clarification.

If the procedure was determined to be necessary and established on the first visit, then I would think the EM visit would be appropriate for that visit. However for the 2nd and 3rd encounter, unless other significant and separate issues were addressed apart from the perfusion event, there really is no indication for billing an EM service. This is my understanding as we have faced the same issue when billing for pre-scheduled infusion treatments. EM codes would not be appropriate for the 2nd and 3rd encounter.
