Dx states: C/D Asymmetry
I haven't been able to find this Dx.
My doctor comes up with 743.57 congenital anomaly of optic disc.
Can someone please give me some clarification on this and let me know if this is the right code for this?

Coloboma of optic disc shows excavation within the optic disc. It is one of the conditions of congenital anomalies of the optic disc.
This can produce C/D asymmetry which can affect the visual acuity.
I feel that your doctor is right who has a broader outlook with the signs and syptoms the patient has been presenting with.
Along with the congenital code he suggested, if the patient is having a visual acuity problem/defect , you can also give one from 368. 40- 368.45.

Look at 365.01 for asymmetric cup-to-disc ratios.
 
Good catch Majo! I had been thinking of glaucoma and C/D asymmetry as an aquired in adults. But since there is no documentation by the physician with glaucoma diagnosis, Physician who saw the patient relates in terms of congenital anomaly, an dno mention about glaucoma I got out of it because this cod eneeds glaucoma to be documented Right?
But your catch is a good catch and hope it fits well with the case.
