Hello,
I could use some help here with diagnosis. Maybe its late in the day on a Friday and I'm not seeing it. Is this a late effect or is it infectious arthritis or, an arthropathy. Just point me in the right direction. The patient is a high performance athlete who had an infection as a 5 yr old child. It required open debridement several times at that time. Since then, this is the condition he is being seen for now.

Here is the note:

PREOPERATIVE DIAGNOSIS: Right knee postinfectious arthritis/ adhesions/synovitis and loose bodies.

POSTOPERATIVE DIAGNOSIS: Right knee postinfectious arthritis/ adhesions/synovitis and loose bodies.

PROCEDURE:
1. Right knee arthroscopic chondroplasty.
2. Right knee loose body removal.
3. Right knee lysis of adhesions.
4. Right knee synovectomy.


ARTHROSCOPIC FINDINGS:
1. Severe arthrofibrosis and adhesions throughout the patellofemoral, medial and lateral gutters and anterior interval.
2. Grade III and IV changes throughout the lateral compartment with several loose bodies in the joint there.
3. Mild grade I and II chondromalacia on the medial compartment as well as the patellofemoral compartment.
