I am new to pathology coding. I work in a ASC for gastroenterology.
Please help me with the following. We use very limited number of dx codes d/t the specialty so once I get these few down I should be ok. Thanks in advance.
Gastric Nodule Bx;
Focal elongation of the gastric pits and focal fibrosis of the lamina propria suggestive of hyperplastic like changes. Is this a gastric polyp? 211.1?

GE junction BX:
gastric body type mucosa with mild increase in lymphocytes and plasma cells. 530.11??
 
I do coding for a pathology group. I agree with the codes suggested. Just an additional note about V12.79 for history of Barrett's - this is the correct code, but our experience is that Medicare will not accept this as a stand-alone code. In this particular case, you should be fine because you have the additional code of 537.89. If you have a case for Barrett's surveillance and the biopsy is normal, you can use the code for Barrett's (530.85) instead of V12.79.
