Can anybody help clarify this for me? I believe I heard a speaker at a conference I attended mention Medicare now only wants us to code for the diagnosis that we are actually treating the patient for, and not to include diagnosis for the other conditions that factor into our medical decision making but that we are not actually treating during the visit......did I understand this correctly? Is there documentation somewhere from Medicare regarding this? Can we not code for the diangosis that factored into the providers medical decison making if documented in the visit record?
