Pt previously had stroke and now comes to the clinic for anti coauglation maintenance.What is the code I shoud use when there is not sufficient documentation about residual condition?

Patient without any residual condition I use V58.83, V58.63 & V12.54 .

When do you use 436?

436 is only used in the event that a patient is having a stroke at this very moment!
