Wiki Dshs rejecting child visits in psychology

I need help please. I'm new to billing for a Psychology office and they see children. I'm in Washington state billing DSHS for this but they are getting rejected because something is missing off the electronic claims. Can someone tell me what is required on a CMS1500 electronic form to bill this visit?
