Wiki Dry Eye MGD Treatment Payer Policies

We are working to update our payer policies for various dry eye tests and procedures. Does anyone have links to good resources to find investigational policies and coverage policies? We are starting with Medicare, BCBS MN, Health Partners, and Medicaid MN. But any insurances you have would be great to give us resources.

Also looking for what treatments and tests your office uses (we are making a cross reference list of what is similar and what is different when applying the CPT codes in case our Doctors add something new.) Thanks much!

Be careful with certain insurers. Some will say they cover certain procedures, such as Lipiflow, but their reimbursement may not even cover the cost of the materials to do the procedures.
