How would you handle a situation when you know that the time the physician is documenting is not correct. Ex: documents that the visit was from 10-10:55 but it was really 10-10:30. Thanks
Documenting that the visit was from 10:00 to 10:55 does not qualify for counseling time. The physician must specify that more than 50 % of the visit was spent on counseling and must document the accurate total time and total time spent counseling. You will need to ask for clarification from the physician.
