I have a surgeon that documents the colon polyp site location in centimeters. I have a cheat sheet that is averages that I use. He will say Ascending colon 96 centimeters. Which doesn't come near what is on this sheet. When queried he states he doesn't know the anatomical site. I'm at my wits end here. Any advise would be appreciated. I've attached the cheat sheet I use.

No need to measure but ensure that when polyp is dissected give correct location of intestines . The intestine/colon has differ sections in dx block of D12 . If lab results positive use dx C7A or C17-C18 blocks per area. I would not use dx K63 and dx D12 or C7Aor C17 same time on claim too.Lab test usually for these polyp removal is 88305.Add modifier XS if polyps removed in differ place in colon too
