I work in DME Land and know that surgical dressings with collagen are covered by Medicare when medically necessary but only for weekly changes (4 allowable monthly). Does anyone have a great example of the kind of documentation Medicare is accepting to support DAILY or QOD use instead? Their verbage "Reasonable and Necessary" is VERY vague. It's unclear what their idea of reasonable is!!!
